Record integrity

The reliability and accuracy of information stored in a medical or operational record.

Record integrity is the reliability and accuracy of information stored in a medical or operational record. A record has integrity when it is complete, unaltered unless authorized, and traceable to a valid source. In cyber security, this matters because records drive decisions, billing, escalation, compliance, and patient care. If an attacker, buggy integration, or careless workflow changes a note, timestamp, dosage, or status field, the damage can persist long after the original mistake.

Record integrity can be weakened by unauthorized edits, malformed imports, copied AI-generated text, or hidden automation that writes into the system of record. Defenders protect it with access controls, audit logs, change review, validation rules, backups, and human approval for high-impact updates. In practice, the goal is to ensure that what appears in the chart or operations database is trustworthy enough to act on, investigate, and defend in court or during an incident response.
