Governance fragmentation

A situation where responsibilities are spread across multiple actors without a clear control chain.

Governance fragmentation is a situation where responsibility for a system is split across multiple teams, vendors, or managers without a clear control chain. In cyber security, that makes it hard to know who approves risk, who monitors changes, and who has authority to stop a process when something looks wrong. The result is often weak accountability, slow escalation, and security decisions that are easy to defer but hard to trace.

Attackers can benefit from fragmented governance by exploiting unclear ownership, delayed handoffs, or gaps between technical and business oversight. Defenders reduce the risk by assigning a single system owner, defining escalation paths, keeping change and access logs, and documenting who can freeze deployment or override outputs. In AI, biometrics, and other high-impact systems, governance fragmentation can leave a model running while no one can prove who reviewed it, who signed off on it, or who is responsible when it misbehaves.
