Major nonrecoverable fault

A controller fault state that stops normal operation and requires a power cycle to recover.

A major nonrecoverable fault is a controller error state that stops normal operation and cannot be cleared by software alone. In affected industrial controllers, the device enters a locked fault condition and must be power-cycled to resume service. This is more severe than a temporary alarm because it interrupts control logic, communication, and any processes the controller is running.

In cyber security, this matters because attackers do not always need to steal data or change logic to cause damage. A malformed industrial protocol message, such as bad CIP traffic, can trigger the fault and create a denial-of-service condition. For defenders, the key controls are firmware patching, protocol exposure reduction, segmentation, and careful validation of controller models and versions. In OT environments, recovery may require maintenance windows, safety checks, and manual coordination, so a single fault can become costly downtime.
