Confirmed access

evidence that an attacker has already entered a victim environment.

Confirmed access means there is evidence that an attacker has already entered a victim environment. This evidence may come from valid account use, authenticated remote sessions, malware beacons, stolen tokens, or traces of internal activity that show the intruder is past the perimeter. It is more than suspicion or scanning: it indicates the adversary has a foothold.

This matters because the defense problem changes once access is proven. Security teams can no longer focus only on blocking entry; they must assume the attacker may already be moving laterally, collecting credentials, disabling tools, or preparing payloads. In real incidents, confirmed access often triggers containment steps such as isolating hosts, resetting exposed accounts, reviewing authentication logs, and hunting for persistence. Rapid action is critical, because attackers may progress from access to data theft or encryption very quickly.
